Background:
Treatment-resistant depression is a severe condition with limited treatment options. Conventional antidepressants often have diminishing efficacy with each successive course of treatment, leading to significant challenges in managing this condition. Psilocybin, a compound found in certain mushrooms, has shown promise as a potential antidepressant in preliminary studies, particularly in patients with life-threatening illnesses. This phase 2 clinical trial aimed to determine the efficacy and safety of synthetic psilocybin (COMP360) in patients with treatment-resistant major depressive episodes.
Summary:
In this phase 2 double-blind clinical trial, 233 participants with treatment-resistant major depression received a single dose of psilocybin (25 mg, 10 mg, or 1 mg control). The primary endpoint was the change in Montgomery–Åsberg Depression Rating Scale (MADRS) total score from baseline to 3 weeks. Results showed that the 25 mg dose significantly reduced MADRS scores compared to the 1 mg dose, while the 10 mg dose did not show significant improvement. Response rates at week 3 were 37% in the 25 mg group, 19% in the 10 mg group, and 18% in the 1 mg group. Remission rates were 29% in the 25 mg group, 9% in the 10 mg group, and 8% in the 1 mg group. The study emphasized the need for clinical vigilance regarding suicidality in future psilocybin trials for depression.
Endpoints:

Limitations:
- Lack of an active comparator.
- Limited ethnically diverse participant sample.
- Exclusion of high-risk individuals for suicide.
- Difficulty maintaining the double-blind structure due to the psychedelic effects of psilocybin.
- Uncertainty regarding the effects of different psilocybin preparations.
The study suggests that psilocybin, particularly at a 25 mg dose, may have potential as a treatment for treatment-resistant depression. However, further research with larger and longer trials, as well as comparisons with existing depression treatments, is needed to confirm its efficacy and safety.
